Women everywhere are applauding Spanx shapewear founder Sara Blakely, who sashayed her way to Forbes magazine's 2012 billionaires list as the youngest female member of this elite club, joining Bill Gates, Warren Buffett, and other business heavyweights.

With dogged determination and a vision (Said Sara to her skeptics, "If we can put a man on the moon, we can make pantyhose comfortable."), she found a way to help us tame our unwanted bulges comfortably so we could look sleek and smooth, at least with our clothes on. For those seeking a better body solution that doesn't require the aid of a compression garment, liposuction is a deservedly popular surgical option to give you a firmer, shapelier, bulge-free figure of which you can be proud when you're wearing swimwear, lingerie, or nothing at all.

In the hands of a skillful plastic surgeon, this procedure can yield a dramatically improved new look with more ideal proportions. Conversely, if too much fat is removed in the wrong places, irregular contours and sagging skin can result.

This leads us to the second category of revision procedures plastic surgeon Dr. Chris Patronella most frequently performs: a proper procedure recommendation was made to the patient, but it was not performed in an effective manner. In the case of liposuction, the skill of the provider is far more important than the method of liposuction that is employed, though persuasive advertising may convey otherwise.

Prior to having surgery with Dr. Patronella, this woman had undergone abdominoplasty and liposuction procedures that left her unsatisfied with the results. Dr. Patronella performed a revision tummy tuck, liposuction of the waist, and fat grafting to the outer thighs to give her more ideal feminine proportions: a narrow waistline with softly-curved hips and outer thighs.

Thankfully, improvement of disappointing results can often be achieved through revision surgery, though sadly this is not always the case. An example of this would be excessive liposuction of the buttocks, which often results in sagging and poor proportions-a difficult problem to fix once the fat is gone.

While all surgery carries the risk that complications could occur, step one in maximizing the likelihood you will safely receive a successful aesthetic outcome is to choose a plastic surgeon who is board certified by the American Board of Plastic Surgery. This indicates the doctor underwent the rigorous training and education necessary to equip him/her to perform plastic surgery procedures skillfully and safely. Accept no less.

Patients typically have a mixture of feelings in the moments prior to having a procedure: excitement and anticipation of their new aesthetic look, but also nervousness about having cosmetic plastic surgery. Afterwards, many express that the professionalism and warm, supportive care of the surgical team made the experience one that is positive and uplifting, rather than scary.

On the day of surgery, anesthesiologist Dr. Bobby Ibarra or another member of the ACPS SurgiCentre anesthesiologist team is one of the first people patients meet in preparation for the surgical procedure. In this interview, Dr. Ibarra gives us a behind the scenes glimpse of the role he plays in supporting our patients and surgeons.

Q: As an anesthesiologist at the ACPS SurgiCentre, what are your goals and primary responsibilities?

Dr. Ibarra: “Our greatest responsibility is to make sure a patient can physically and physiologically tolerate surgery. We are making sure their health is optimal for surgery. A lot of the things we do are related to anticipating and preventing potential problems. We’ve come a long way over the past 20 years—identifying things that make a safe environment and preventing possible problems.

“Our mantra is safety and vigilance: ‘Keep your eyes and ears open at all times. Don’t let anything get the best
of you.’”

Q: Beyond providing anesthesia, you play an important part in helping patients to relax before a procedure. Tell us more about this aspect of your work.

Dr. Ibarra: “The time that I meet with patients during the preoperative evaluation is very important. Beyond any medication that could be given, what most puts patients at ease and calms their nerves is talking to them—telling them what to expect and answering their questions.

“I tell them, ‘I’m going to be there. I’m going to take care of you, and nothing is going to happen.’ Patients are comforted to know a board certified anesthesiologist is watching them at all times to make sure everything goes fine.”

Q: What is one of the most satisfying aspects of your job?

Dr. Ibarra: “It is very rewarding to take care of patients, helping them to safely, painlessly, and unconsciously move through surgery without any problems at all.”

Q: As an anesthesiologist, you have the opportunity to work with many physicians at different hospitals. From your perspective, what makes our surgeons and surgical center great?

Dr. Ibarra: “ACPS surgeons have very high expectations and standards for the anesthesiologists who work here, and they utilize only board certified anesthesiologists to care for patients. Our surgeons have provided the very best equipment at the SurgiCentre; anything we’ve requested—medications and equipment—they have been quick to provide.

“What I appreciate the most is that they really want to have a first class surgery center that meets the highest safety standards and presents a welcoming atmosphere for patients. ACPS surgeons have taken the difficult steps to get the SurgiCentre certified by the American Association for Accreditation of Ambulatory Surgery Facilities (AAAASF)—an organization whose stringent requirements for equipment, operating room safety, personnel and surgeon credentials are considered the gold standard for ambulatory surgical facilities.

"That accreditation really puts the ACPS surgical facility above a lot of small surgery centers. I don’t have to worry when the Texas Department of Health comes in unannounced because I have the reassurance of knowing that we will pass every test.”
Revision Surgery—The Main Causes Behind It, Part I

		Before Surgery

This patient consulted with Dr. Patronella because she was unhappy with the results of a liposuction procedure she received elsewhere, as shown in this photo taken prior to the revision procedure she had with Dr. Patronella. This particular treatment left her with contour irregularities and more loose skin. Based upon Dr. Patronella's recommendation, she elected to have a tummy tuck, liposuction, and fat grafting to restore a smooth, natural appearance to her tummy.

When done right, cosmetic plastic surgery can produce enormously satisfying aesthetic results and a corresponding boost in self-confidence. Sadly, a growing area of my practice has become revision surgery to correct the disappointing results of cosmetic procedures gone wrong.

Most of the problems I see fall into one or more of the following categories:

The provider demonstrated an error in judgment, recommending a procedure for which an individual is not a good candidate.

A proper recommendation was made, but the procedure was ineffectively performed.

The procedure is properly performed, but a significant complication occurred during the postoperative period.

In this article, I’ll focus on the first issue: a poor aesthetic outcome due to an improper procedure recommendation. As an illustration of this, some of my revision patients, desiring to restore a firmer pre-pregnancy appearance to their abdomens after having children, were told that liposuction would effectively improve their concerns. Unfortunately, this treatment plan only made their abdomens look worse, leaving behind even more loose skin and / or contour irregularities.

For most of these patients, a tummy tuck would have been a better fit. In this procedure, the loose skin is removed and the weakened abdominal muscles are tightened to restore a flat, smooth look to the abdomen. In many of these cases I incorporate liposuction, both to give the waist a narrow, elegantly tapered appearance and to improve the proportions of the hips, thighs and buttocks and other areas where excess fat commonly accumulates. While liposuction alone would have been insufficient to correct their concerns, it served as an excellent complement to the primary procedure.

In some cases—these patients sought out procedures advertised as “less invasive” in the hopes of achieving the improvement they desired with less expense and downtime. For the right patient, nonsurgical cosmetic procedures such as CoolSculpting® to reduce fatty bulges may be a good fit to help them achieve their goals. But for loose skin and more significant contouring needs, a surgical procedure that includes the removal of excess skin is typically required to obtain a desirable aesthetic outcome.

<itunes:subtitle>Revision Surgery—The Main Causes Behind It, Part I</itunes:subtitle>
			<itunes:summary>

In this article, I'll focus on the first issue: a poor aesthetic outcome due to an improper procedure recommendation.

In some cases-these patients sought out procedures advertised as "less invasive" in the hopes of achieving the improvement they desired with less expense and downtime.

Dr. Patronella's True Form Tummy Tuck is a sophisticated abdominoplasty method he developed to better capture the abdomen's natural beauty. He incorporates techniques to redefine the subtle contours and smooth, elegant lines that make the abdomen one of the most attractive features of a youthful, fit and healthy body.

He was compelled to design a new approach to abdominoplasty because of his dissatisfaction with the results typically produced by the traditional tummy tuck, namely, an overly flat appearance that had none of the soft contours that can be seen on the natural abdomen. In addition, the procedure often left patients with unattractive characteristics that were the obvious results of surgery: a taut lower abdomen that visibly contrasted with looser skin above the navel and Read more...

<!--

Challenged to create surgical results that more authentically reproduced the physical attributes of the naturally attractive abdomen, the surgeon studied photos of youthful, attractive abdomens, identifying the common features that characterized them and contributed to their overall beauty. Gradually, he began incorporating small steps into the tummy tuck procedure to recreate these characteristics, using techniques that were typically reserved for other areas of the body.

His endeavor met with resounding success, sending patient satisfaction rates soaring, and inspiring the launch of a website devoted exclusively to his True Form Tummy Tuck™—a procedure that represents the culmination of his efforts to perfect tummy tuck surgery.

In the procedure, Dr. Patronella not only redefines the abdomen’s soft anatomical nuances, he also focuses on creating smooth transitions between all areas, eliminating the most common abdominoplasty pitfalls he sees among his revision patients: an abdomen with looser skin above the belly button and tight skin below it, and an odd-looking “step-off” where the tucked skin meets the pubic hairline—an issue that results when there is an uneven tissue thickness between the areas above and below the incision line.

To visually complement the abdomen’s smooth new contours, the surgeon uses contouring techniques to give the waist more slender proportions and a graceful, longer look that tapers toward the waist.

The waist and hips are a critical element in ensuring the abdomen has an attractive appearance,” noted Dr. Patronella. “If you create a nice abdomen, but you are inattentive to the appearance of the waist, it will be obvious that the look was surgically created.”

It’s a surgery that often produces a transformation in self-image that is as impressive as the aesthetic change it creates. For Dr. Patronella, that is the greatest reward and the one that reflects the measure of his success in achieving his original goal.

He noted, “While no plastic surgeon can perfectly duplicate what is naturally created, I believe this procedure closely captures and restores the simple, sensual beauty and elegance of the abdomen in its true form.”

For this 61-year-old woman who desired a more refreshed facial appearance, Dr. Patronella performed an SMAS facelift, necklift, blepharoplasty (eyelid surgery), and a browlift. In facial rejuvenation procedures, Dr. Patronella often combines surgical and nonsurgical treatments. In this case, he used a hyaluronic acid cosmetic filler to add fullness to her lips and to soften the nasolabial folds that stretch from the bottom of the nose to the corners of the mouth.

When it comes to facial rejuvenation, Dr. Patronella has found that while most individuals desire to correct the characteristics of facial aging which cause them to appear tired and worn, they often are concerned that cosmetic plastic surgery will alter their unique facial characteristics.

“Most of my patients say, ‘I don’t mind my age. I just want to look good for my age,’” said Dr. Patronella. “Creating a naturally rejuvenated look that preserves the unique personal characteristics of an individual’s face is the most important thing I can do for a patient.”

To create natural-looking results, said Dr. Patronella, there must be an equal distribution of lift applied to each area of the face, as pulling one area excessively leads to an artificial look. In addition, because most people experience a loss of bone and deep tissue both in quality and quantity, restoration of this lost tissue through fat grafting is a key component of restoring a youthful look to the face. This factor, he said, is the likely reason for the much publicized examples of windblown-looking celebrity faces. “They get repeated pulls without restoring the volume.”

Creating natural-looking facelift results requires an attentive eye to a myriad of other details, as well, in order to avoid producing a look that reveals the tell-tale signs of surgery. These include making sure the earlobes are not pulled, ensuring that there is a small concave area in front of the ear, and avoiding pulling up the hairline.

Dr. Patronella is particularly attentive to placing the incisions in such a way that they're hard to detect, angling his incisions in the natural crease of the earlobe and behind the ear. "My goal is to make the incision in such a way that my patients can wear their hair up without feeling self-conscious," he said.

While the late 40s to the early 50s is the most common age range of Dr. Patronella’s facelift patients, the surgeon said he has seen patients in their 30s who need facelifts and those in their 50s who do not. “The aging process can affect people in different ways. We must be flexible in our thinking and not confine our approach based on age alone.”Factors such as genetics, lifestyle, major weight changes, and nutritional habits can all accelerate the aging process, he said.

Though patients often focus on one or two aged features such as jowls or loose neck skin, most will also have other aged facial features such as fine lines and wrinkles around the mouth, that, if unaddressed, will lead to an unbalanced look that appears partially rejuvenated and partially aged, said Dr. Patronella. To avoid this, he evaluates each aging facial feature in order to advise patients of the best approach to achieving rejuvenation that appears balanced, thorough, and natural.

An Interview With Aesthetic Nurse Specialist, Abby Loner, RN, BSN

	By Sherri Roberts Lopez

As Dr. Patronella’s aesthetic nurse specialist, Abby Loner, RN, specializes in providing nonsurgical cosmetic injectable treatments to restore a more youthful look to the face. While cosmetic injectables are a fast and easy way to smooth away lines and wrinkles and plump the lips, with all the great options available, it can be perplexing to know which one to choose. ACPS Communications Director Sherri Lopez sat down with Abby to discuss this and some of the other most commonly asked questions about cosmetic injectables.

Sherri: Abby, how do you develop an individualized cosmetic injectable treatment plan that will provide the most effective rejuvenation benefits for a patient?

Abby: I like to look at the anatomical structure of the face. I don’t use a cookie cutter approach because each person’s facial structure is unique. I listen to their concerns—what they see that bothers them—and what they would like to achieve. I consider both their goals along with the anatomical characteristics that I see in order to determine which injectable will provide them with the most benefit.

I explain the limitations and expected results. And we set up a plan that is based upon their goals, budgets, and plans. For example, if a patient has an upcoming event for which she wants to look her best, we plan the injectable treatment within a time frame that will allow any bruising that may result to be diminished.

Sherri: What is the difference between BOTOX® Cosmetic, Dysport™, and hyaluronic acid-based fillers such as Restylane®, Perlane®, and Juvederm™?

Abby: BOTOX®Cosmetic and Dysport™ are used for lines that are formed as a result of muscle movement, generally in the upper third of the face. They are quite effective to soften lines around the outer corners of eyes, the vertical lines sometimes called “elevens” that form between the brows, and the horizontal forehead lines that can give the face a tired, worn look.

Fillers are used for deeper lines and folds, for the most part, in the bottom third of the face. But they can also be used to add volume to the face and lips. I choose the filler that will provide the most effective results based upon the location of the area being treated and/or the depth of the wrinkles.

Sherri: With BOTOX® Cosmetic and Dysport™, how do you achieve the balance between softening lines without giving someone a frozen look that appears artificial?

Abby: The anatomy of the face is very complex. You have to inject BOTOX® Cosmetic and Dysport™ into very specific muscles in order to gently diminish lines without taking away all the expressive features of the face.

In the same way, with fillers, it is important to have an understanding of the various depths of the skin because different fillers are put in at different depth levels. The goal with both kinds of treatments is to create a look that is naturally youthful, rather than overdone.

Sherri: How long does it take to see the results of cosmetic injectables, and how long do they typically last?

Abby: With hyaluronic-acid based fillers, the results are immediate, but you may not see the final result until up to a week after treatment. For most people, they last about 6 months to one year.

For BOTOX® Cosmetic and Dysport ™ treatments, it typically takes about one week for the lines created by muscle movement to be smoothed out. Their benefits typically last about three to five months.

Sherri: What are some of the most common potential side effects of these treatments?

Abby: After treatment with BOTOX®Cosmetic or Dysport™, some people may have a little bit of redness at the injection sites for about 20 minutes. With fillers, bruising and swelling may occur, though this varies between individuals. The best way to decrease the chances of bruising and swelling is to avoid alcohol for 24 hours before and after treatment. In addition for 24 hours before and after treatment, avoid taking Alleve, aspirin, ibuprofen, fish oil, flax seed oil, and Vitamin E.

Sherri: While lines and wrinkles start to become more noticeable to most individuals when they are in their 30s and 40s, you do have patients in their 20s who choose to have treatments. How can this be beneficial?

Abby: BOTOX® Cosmetic and Dysport™ actually provide a dual benefit. By gently relaxing the muscle movement, they correct lines that have formed. But, they also work as a preventive measure, keeping the lines from deepening.

Sherri: What are some of the less commonly known benefits that injectables can provide?

Abby: Some people don’t realize that BOTOX® Cosmetic and Dysport™ can be used to give you a little bit of a browlift. They can be injected in a way to relax the muscles that depress the brows and cause them to sag. This gives the eyes a more lifted, awake look.

They can also be used to decrease underarm sweating. This is an especially popular treatment around the warm and humid months of the wedding season. The treatment takes about 20 minutes and typically lasts about six months.

Sherri: I’d like to ask you about one of the newer treatments that is becoming increasingly popular and receiving a lot of attention in the media: Sculptra®Aesthetic. How is it different from other injectables?

Abby: Sculptra®Aesthetic stimulates collagen to replace volume that has been lost in the face. As part of the aging process, the face experiences a loss of fat, bone, and collagen. This is why the cheek pads often develop a more flattened appearance, and the lower lids become more sunken. There are a lot of subtle contour changes in other areas, as well, such as the temple region, which often become more hollow in appearance, and along the jowls. Sculptra helps to restore youthful-looking volume to these areas of the face.

Unlike hyaluronic acid-based fillers, Sculptra®Aesthetic’s results are not instantaneous. They appear gradually through a series of three treatments spaced about four to six weeks apart. The full results are usually evident in six months and can last for up to two or two and a half years, on average.

Sherri: It’s a cruel tradeoff, but often people who are fit and regularly exercise can develop an older, more gaunt facial appearance because they have less facial fat, while their bodies appear more youthful because they are in great shape.

Abby: Yes, that’s the great thing about Sculptra®Aesthetic and other cosmetic injectables. They restore volume to the face so you can have a youthful-looking face and body!

Sherri: What do you find to be the most satisfying aspect of your role as an aesthetic nurse specialist?

Abby: I love seeing the instant results and the happiness these treatments bring patients. A lot of them tell me it gives them more confidence because they have a more refreshed and youthful look.

Sherri: Dr. Patronella and many of your patients have told me that what they appreciate about you is that you have a very gentle touch, but you provide very effective results.

Abby: Thank you. I love what I do, and working with Dr. Patronella is wonderful. It’s a true privilege to help people achieve their aesthetic goals, and we have the best patients!

<itunes:subtitle>An Interview With Aesthetic Nurse Specialist, Abby Loner, RN, BSN</itunes:subtitle>
			<itunes:summary>
	By Sherri Roberts Lopez

As Dr. Patronella's aesthetic nurse specialist, Abby Loner, RN, specializes in providing nonsurgical cosmetic injectable treatments to restore a more youthful look to the face.

I don't use a cookie cutter approach because each person's facial structure is unique. I listen to their concerns-what they see that bothers them-and what they would like to achieve.

They are quite effective to soften lines around the outer corners of eyes, the vertical lines sometimes called "elevens" that form between the brows, and the horizontal forehead lines that can give the face a tired, worn look.

Abby: Some people don't realize that BOTOX® Cosmetic and Dysport™ can be used to give you a little bit of a browlift.

Sherri: I'd like to ask you about one of the newer treatments that is becoming increasingly popular and receiving a lot of attention in the media: Sculptra®Aesthetic.

Unlike hyaluronic acid-based fillers, Sculptra®Aesthetic's results are not instantaneous. They appear gradually through a series of three treatments spaced about four to six weeks apart. The full results are usually evident in six months and can last for up to two or two and a half years, on average.

Sherri: It's a cruel tradeoff, but often people who are fit and regularly exercise can develop an older, more gaunt facial appearance because they have less facial fat, while their bodies appear more youthful because they are in great shape.

Abby: Yes, that's the great thing about Sculptra®Aesthetic and other cosmetic injectables. They restore volume to the face so you can have a youthful-looking face and body!

Sherri: What do you find to be the most satisfying aspect of your role as an aesthetic nurse specialist?

Give your face and neck a lift−with Ultherapy! No surgery or downtime required Ultherapy is the only treatment that can lift and tighten sagging skin by targeting the deep tissue layers beneath it...;without cutting or disrupting the skin's surface! Ultherapy, which is FDA-approved for safety, uses ultrasound technology to deliver heat into the deep foundation layer beneath the skin to stimulate the creation of new collagen and the renewal of its existing collagen. The result is firmer skin, and a more youthful, uplifted facial appearance! A treatment can be completed in-office in just 30 to 60 minutes, and afterwards you may immediately resume your normal activities. Though some patients notice initial improvement right away, the greatest toning and lifting is typically seen over the next two to three months as the collagen building process takes place.
The Mommy Makeover

Houston Plastic Surgeon Dr. Chris Patronella specializes in Mommy Makeovers to restore a woman's body after having children While having a child is a cherished and unforgettable experience for most women, pregnancy, breastfeeding, and weight fluctuations take a physical toll upon the body that leaves many women with breasts that have loss fullness and firmness, stretch marks, loose abdominal skin, and a tummy bulge that won't budge despite diet and exercise. These changes frequently take a psychological toll as well, causing a woman to feel self-conscious about her body and hesitant to wear the figure-accentuating jeans, tops, dresses, and swimsuits she may have once enjoyed. For Dr. Patronella, being a participant in the restoration of a woman's body to its pre-pregnancy state is particularly rewarding.
